Frustrated By Your Fillers? You're Not Alone!

Non-invasive cosmetic procedures are at an all-time high, and with so many choices on the market, confusion also prevails.  Let us look at the top filler offered by the Neigel Center for Cosmetic and Laser Surgery.

Filler Type Lasting Effect
Restylane® & Juvederm Ultra™ Hyaluronic Acid Gel 4-6 Months
Perlane® & Juvederm Ultra Plus™ Concentrated Hyaluronic Acid Gel 6-9 Months
Radiesse® Calcium Mircospheres 12 Months
Sculptra® Poly-L-lactic acid Acid Gel 24 Months

Hyaluronic acid is a natural sugar found in all living cells, and is a main building block of collagen and elastin, both of which are necessary for youthful-looking skin.  Poly-L-lactic acid becomes lactic acid in the skin.  (When we exercise, and “feel the burn”, it is the lactic acid in our muscles that cause this sensation.)  Upon injection, lactic acid stimulates the skin to generate more collagen, which in turn fills in lines and wrinkles.  Essentially, your own body is producing a face-lift!  Finally, calcium microspheres are simply small beads of calcium suspended in a natural gel.  Like hyaluronic and lactic acids, calcium boosts collagen production in the skin for a wrinkle smoothing effect.

As you can see, all fillers used at the Neigel Center are naturally based, and therefore safe for everyone, with no testing required.  Furthermore, you will be pleased to know that fillers can be used for much more than nose to mouth lines.  Fillers have helped thousands of clients improve the looks of jowls, hollow cheeks and temples, under-eye bags and dark circles, earlobes, and even hands.

Get The Facts About BOTOX® Cosmetic

Botox®, or Botulinum Type A, a natural purified protein, given as an injection, relaxes facial muscles that are affected by our expressions.  The skin that overlies the area where the Botox® is given will smooth out, diminishing wrinkles, giving a more relaxed appearance.  Botox® has been given medically for years for such patients with ophthalmic and neurological conditions.  It is FDA approved for selective cosmetic reasons.  It is one of the most common and popular treatments for wrinkle reduction.  Botox® is a very safe and simple procedure; a very small amount of Botox® along with a very tiny needle make that so.  A few tiny injections can be given to the glabellar (area between the eyebrows), the horizontal forehead lines, and the crow’s feet, which are the usual areas of concern.  It can be used on the jowls, neck, lipstick lines and lower face.  It is also used for the relief of migraines and excess sweating.  You can resume normal activities after the treatment.  Dr. Neigel is a National Trainer of Botox® Cosmetic.  To-date, she has injected thousands of patients.  Dr. Neigel attends yearly conferences and training on the latest techniques for this procedure.

Liquid Face Lift

The Liquid Facelift is the best alternative to a surgical facelift, particularly if you’re either uncomfortable with the idea of surgery or you cannot undergo surgery for health related reasons.  The liquid facelift is also a wonderful option if you’re considering surgery down the road however want to see what results might look like before making the surgical commitment.

This procedure is a combination of both immediate reacting and gradual reacting injectable fillers that, when used collectively and artistically throughout the face, create a gorgeous fullness and lifting effect.  One of the filler components used is a product called Sculptra® Aesthetic, a gradual building and thickening filler that takes its full effect in about two months.  This filler is one of the most genius products on the market today.  Lasting approximately two plus years, this filler fills out hollows in the face due to aging or weight loss, hollows under the eyes, and lifts the cheekbone area, ultimately reverses the sagging effects of gravity due to aging.  On the more immediate side, fillers such as Juvederm™ and Restylane® are used in areas such as the laugh lines and around the mouth, restoring a plumpness reminiscent of your youth.  These fillers range in how long they last eight months to one year depending on which fillers are selected.  These fillers are combined with either Botox® or Dysport, the two anti-wrinkle injectables which diminish the frown lines in between the eyes, the crows feet around the eyes, and the lines that form across the forehead.  These last roughly four months four to six before needing a top up injection.

The overall effect of this treatment is literally a liquid facelift, whereby the skin is more taught, more full and significantly diminishes the folds and wrinkles that unfortunately come along with aging.

Perlane
What is Perlane?
Perlane, like Restylane, is a dermal filler and is a long-lasting gel that, once injected into the skin with an ultra-fine needle, lift and add volume and help diminsh fine lines. Made of a natural substance called hyaluronic acid, it is a completely safe treatment. The hyaluronic acid in Perlane simply supplements the body's age-depleted hyaluronic acid to plump-up the lip or to raise the skin and 'fill in' the particular wrinkle being treated. The difference between Restylane and Perlane is the size of the particle (with Perlane containing more hyaluronic acid). This means that Perlane does a better job of filling deep lines while Restylane is used in fine lines or in thin skinned people. These products last for between six and nine months, although individuals will have varied results. Perlane is an excellent treatment for wrinkle correction, lip enhancement, smile lines and deep folds.

Restylane®

When we are young, our skin is smooth and elastic, with a healthy glow.  As we age, our ability to maintain that smoothness and elasticity diminishes, resulting in skin that becomes drier, thinner, and less able to replenish itself.  Our body’s natural collagen and hyaluronic acid levels decrease and our skin becomes looser.  This leads to our skin wrinkling and appearing older than it should.

Approved by the FDA, Restylane® is designed to match your skin, smoothing wrinkles, sculpting lips, or shaping facial contours.  With its unique ability to bind with water, Restylane® is able to perform for many months in your skin.  The most common areas for a Restylane® treatment are the glabellar lines (between the eyebrows) and the nasolabial folds (from the root of the nose to the angle of your mouth) under the eyes to disguise the bags and crow’s feet.  When used for lips and facial contouring, Restylane® will add shape and volumes to the area.  Restylane® is able to create volume under your wrinkles, which are then lifted and smoothed out.

Since no pre-testing is required, a Restylane® treatment can be performed almost immediately.  It usually takes about 30 minutes depending on the type of correction you want.  Most people find the injections relatively painless, but a topical anesthetic can be applied to the area prior to your treatment.  You will be able to see the results as soon as your treatment is completed.

Sculptra® (Liquid Face Lift)

Facial wrinkles, creases and facial fat loss (lipoatrophy) can give a clear indication of your age and sometimes can give you the appearance of being older than your natural age.  Sculptra® has been clinically proven to be long lasting and well-tolerated.  It is a synthetic injectable material made from microscopic crystals of Poly-L-lactic acid (synthesized simple sugar).  This material does not harm the body, is able to be broken down by the body, and has been widely used for many years in dissolvable stitches, soft tissue implants and other types of implants.  No skin testing is required prior to use.  Sculptra® is injected below the surface of the skin in the area to be treated.  It provides a gradual increase in skin thickness.  Sculptra® is used to restore volume to the sunken parts of the face; filling in the hollow under the eyes, heightening cheekbones and reshaping the chin area.  Its use is similar to the fat injections to the face, without the two to three weeks of swelling.  Visible results appear a few weeks after your first treatment.  Unlike fillers, Sculptra® is more like a fat graft; it causes a reaction in the skin, which forms new collagen.  Sculptra® requires a series of two or three treatments to achieve adequate filling for your best results.  Results will differ for each person.  In a clinical study treatment results lasted approximately two years.  Touch-up treatments may be needed to maintain the desired effect.  Short of undergoing a facelift or laser resurfacing, which are both surgical procedures that work beautifully, Sculptra® is a quick non-surgical approach to adding volume to your face.  Many patients consider this option for various reasons.  Volume.  Duration.  Safety.

Hand Fillers - The Matter At Hand

We may devote countless hours to complexion perfection, but one quick glance at our hands reveals the truth.  Of course, there is nothing wrong with mature hands that proudly signify a lifetime of dependable use.  On the other hand (pun intended), there is nothing wrong with not wanting to display the signs of use: dark spots, wrinkles, and crepe-like consistency.  The same anti-aging treatments available for the face can be used on the hands to achieve similar results.  For example, hyperpigmentation, usually referred to as dark spots, is the most common complaint.  A combination of hydroquinone (a skin lighting agent), chemical peels and laser sessions can effectively reduce or eliminate this problem.  Another frequent objection is fat loss, which causes veins to become more apparent.  This condition is easily altered with the injection of fat or fillers such as Radiesse®, Restylane®, Juvederm™ and Sculptra®.  Once the hands regain plumpness, it is time to refine their tone and texture with exfoliating and rejuvenation treatments such a microdermabrasion and Gentle Waves.

You may still be wondering which treatments are right for you.  The best way to answer this question is to schedule a consultation.  Everyone is unique, and requires a combination of fillers in specific amounts.  Rest assured that your doctors will pinpoint your individual needs and provide instant beautiful results.

How is the procedure done?
Fat transfer is performed in our office on an out-patient basis.  Both the area from which the fat is taken and the treatment site are anesthetized with a local anesthetic.  Using a small needle attached to a syringe, fat is removed from a donor site where your fat is most tightly packed, such as your abdomen, or buttocks.

Once removed, your fat is processed to remove excess fluids and then re-injected using another needle, which is placed just under your skin beneath the wrinkle.  This process may be repeated until the desired correction has been achieved.

Harvesting the fat cells for re-injection is often performed in conjunction with other procedures, such as a liposuction, tummy tuck or cosmetic breast surgery.  When fat is harvested during another procedure, it is often possible for your surgeon to harvest a larger amount of fat, then freeze and store your fat for future injections when they become necessary.
